Registration 

To apply for a Golden Buckeye Card, bring appropriate ID to the Service Desk. Ohio residents that are 60 years of age and older will need either a drivers’ license, birth certificate or state identification that verifies their age. Ohio residents that are totally or permanently disabled must have a document that states their condition as totally or permanently disabled as well as identification with proof of birth date.
